Form 1 vs Form 1A
Form 1 is a self-declaration; Form 1A is a medical certificate. They are not interchangeable. Parivahan's current guidance states that Form 1A applies in specified cases, including applicants aged 40 or above for the referenced new-DL workflow, while the renewal guidance also identifies transport-vehicle and age-based applicability. The application itself should decide what is requested.
Do not upload a blank Form 1A. The required sections must be completed and certified in the prescribed way.
Form 2 and Online Applications
Form 2 contains fields and service choices for learner licence, driving licence, addition of vehicle class, renewal, duplicate, and change/correction of address or name. However, downloading Form 2 does not create an application number, pay the fee, reserve a slot or submit the service.
Start or continue the actual transaction through Sarathi when the state offers it.
Blank Form 3 vs Printed Learner Licence
The official blank Form 3 shows the prescribed format. An applicant who needs an issued LL must use Print Learner Licence (Form 3) after approval. Typing personal details into the blank template does not create a valid licence.
File and Upload Problems
- Download the PDF again from the official source if it is corrupted.
- Use a current PDF reader and avoid online “unlock” or conversion sites for documents containing personal data.
- Scan completed forms clearly and in the file type/size requested by Sarathi.
- Keep every edge, signature, photograph and certification visible.
- Use Upload Document/Re-upload for the same application when an objection is raised.
- Never edit an authority-issued form to remove declarations or change wording.
Frequently Asked Questions
Is there one form for every DL service?
No. Form 2 is multi-purpose, while medical certification, IDP, surrender and other tasks use their own forms or system-generated records.
Can I submit only the downloaded PDF?
Usually the form is one component of a larger application. The state process can also require online fields, evidence, fee, appointment, verification or test.
Where can I download the eye-test form?
Users often mean Form 1A, the official medical certificate. Use the current official Form 1A and follow the application's medical-certification requirement.
Is Form 7 a smart-card download?
Form 7 describes the licence-card form/specification. It is not a service for users to manufacture or print their own official card.
