Do not upload every document you possess. Submit only the categories requested by the official application.
Proof of Age and Address
Accepted documents are controlled by the current rules and state service. The online form can show approved document types such as specified government identity, passport, birth/education or residence evidence. A document used for address must show the relevant current address, and its name/DOB should not conflict with the application.
If you live at a different present and permanent address, enter each field accurately and follow the evidence rule shown by the selected state.

Form 1 and Form 1A
- Form 1 is a self-declaration of physical fitness.
- Form 1A is a medical certificate completed by the prescribed medical practitioner.
Parivahan guidance identifies age- and category-based situations for Form 1A. Let the live application determine whether Form 1, Form 1A or both are required. A downloaded but uncertified Form 1A is not a completed medical certificate.
File Preparation for Online Upload
- Scan the original clearly in colour when that is requested.
- Include every edge and all pages.
- Use the allowed PDF/JPEG format and size displayed on the upload screen.
- Ensure the name, DOB and address are readable.
- Do not crop stamps, signatures, photographs or certification.
- Name files descriptively for your own records.
- Keep originals for verification when instructed.
What If the Document Is Rejected?
Read the objection text before re-uploading. Common reasons include wrong document category, expired evidence, unreadable scan, incomplete Form 1A, mismatch with the application, missing reverse side or invalid file size.
Use Upload Document → Re-upload/Modify for the same application. Do not create a new application merely to replace a file.
Aadhaar, Mobile Number and Identity Matching
Aadhaar/e-KYC availability does not mean Aadhaar alone satisfies every evidence requirement. It also does not automatically correct the transport record. If name or DOB differs across the application, supporting document and existing DL, determine which source must be corrected before repeating authentication.
Never send Aadhaar, DL, passport or OTP details to an unofficial support number.
Frequently Asked Questions
Are Aadhaar and PAN both compulsory?
Do not use a generic blog list as the rule. The official state/application document selector shows the accepted evidence for that transaction.
Do I need originals at the RTO?
Some workflows require original verification even after upload. Follow the acknowledgement and appointment instructions.
Is Form 1A required below age 40?
It can depend on vehicle category and service. The official application—not age alone—should determine the requirement.
Can I upload a screenshot of a document?
Use the scan/photo standard displayed by the portal. A screenshot can omit security features, edges or readability and may be rejected.
